ABSTRACT

Objective:To survey the status quo of community practice bases for general practice residency training in Wuhan city. Methods:A questionnaire survey was conducted in 12 community general practice training bases affiliated to 11 tertiary hospitals in Wuhan city from March to June 2021. A total of 123 general practice residents and 41 instructors participated in the survey using the different questionnaires. The contents of questionnaires included the basic conditions of the training base and the current status of training at community base, as well as their satisfaction on the program and related problems. The statistical analysis was performed with SPSS software. Results:The conditions of the 12 primary practice bases met the requirements of the Standard for Standardized Training Bases for General Practice Residents (2019 Revision) in terms of size of community hospitals, qualification of the person in charge and faculty team. The survey showed that only 3 bases had psychiatric department; 7 bases had literature retrieval system; all 12 bases had a teaching secretary, but only 1 had a full-time teaching secretary; all 12 bases had established formal assistant relationship with higher-level hospitals, but the frequency of communication was low in some bases. Survey also showed that 68.3% (28/41) instructors were aged 40-50 years; 90.2% (37/41) of instructors participated in epidemic prevention and control; 36.6% (15/41) thought that instructing work was related to professional promotion; 87.8% (36/41) of instructors were generally satisfied with the work performance. Most of trainees (92.7%, 114/123) were satisfied with the training at the primary base, and there was a difference in satisfaction degree between trainees with different enrollment sources (χ 2=6.92, P=0.032). The proportion of trainees with confidence for future career increased from 39.8% (49/123) before training in community bases to 60.2% (74/123) after training. Conclusions:The survey shows that primary practice bases for general practice residency training in Wuhan have met the basic requirements; however, the evaluation and motivation of teachers need to be improved, the opportunities for trainees to practice in general outpatient clinics should be increased, research and teaching need to be emphasized, and the management of trainees needs to be strengthened.

ABSTRACT

OBJECTIVE:To investigate the effects and mechanism of puerarin (Pue) on hypoxia-induced pyroptosis of pulmonary artery smooth muscle cells (PASMCs). METHODS :PASMCs of rats as research objects were randomly divided into normoxia group ,hypoxia group and hypoxia+Pue group (0.2 mmol/L). Except for normoxia group ,other groups were cultured with 5% CO2 and 3% O2 at 37 ℃ for 24 hours to establish hypoxia model. Western blot assay was used to detect the expression of pyroptosis related proteins [NOD-like receptor protein- 3 (NLPR3),caspase-1,interleukin-1 β (IL-1 β),apoptosis-associated speck-like protein (ASC)]. Lactate dehydrogenase (LDH)release assay was used to detect the release of LDH in cells ;Hoechst 33342/PI double staining test was adopted to detect the proportion of pyroptosis positive cells. PASMCs was randomly divided into normoxia group+control plasmid group ,hypoxia+control plasmid group ,hypoxia+over-expression plasmid group and hypoxia+ over-expression plasmid+Pue group. Except for the normoxia+control plasmid group ,the other groups were established hypoxia model by the same method. After transfection of control plasmid or NLRP 3 over-expression plasmid ,Western blot ,LDH release test and Hoechst 33342/PI double staining test were used to investigate whether Pue could inhibit hypoxia-induced PASMCs pyroptosis by interfering with the activity of NLRP 3 inflammasomes. RESULTS :Compared with normoxia group ,the expression of pyroptosis related proteins ,the release of LDH and the proportion of pyroptosis positive cells were increased significantly in hypoxia group (P<0.05 or P<0.01). Pue had the effect of reversing the above indexes (P<0.05 or P<0.01). When the NLRP 3 inflammasome was over-expressed ,the expression of pyroptosis related proteins ,the release of LDH and the proportion of Δ 基金项目:黑龙江省自然科学基金资助项目(No.ZD201416) pyroptosis positive cells were increased significantly (P<0.05 *教授,博士生导师 ,博士。研究方向 :心血管药理学 。电话: or P<0.01). Pue could inhibit the above phenomenon through 0451-58853046。E-mail:zhangxd85@163.com regulating NLRP 3 inflammasome (P<0.05 or P<0.01). 中国药房 2021年第32卷第11期 China Pharmacy 2021Vol. 32 No. 11 ·1337· CONCLUSIONS:Pue can significantly inhibit the hypoxia-induced pyroptosis of PASMCs by down-regulating the expression of pyroptosis related proteins ,reducing the release of LDH and proportion of pyroptosis positive cells. The mechanism is related to the activity inhibition of NLRP 3 inflammasome.

ABSTRACT

Objective To construct a CaME141G fusion protein-expressing plasmid, and to express, purify, and identify the activity of the recombinant protein. Methods The 141st site of the wild type CaM,E (GAG),was mutated to G (GGG),using site-specific mutagenesis technology. Escherichia coli BL-21 was transformed with the mutant plasmid. The GST-CaME141G fusion protein was mass-cultured and induced for expression. Subsequently, the GST-CaME141G fusion protein was purified using GS-4B beads. PreScission protease was applied to remove the GST,the Bradford method used to determine the concentration of purified protein, and SDS-PAGE used to detect its relative molecular weight and purity. The GST pull-down assay was used to study the protein's biological activity. Results The CaME141G protein was successfully purified at a high concentration and purity. The protein could interact with PreIQ protein fragments from the myocardial CaV1. 2 calcium channel C terminal,in a CaME141G concentration-dependent manner. Therefore,CaME141G has the ability to bind with the CaV1. 2 calcium channel. Conclusion This study successfully constructed a CaME141G fusion protein-expressing plasmid and purified the CaME141G protein. This lays a foundation for regulating the function of CaM mutations in the myocardial CaV1. 2 calcium channel, and for the study of its relationship with diseases of the cardiovascular system.

ABSTRACT

Sulfur fumigation is a traditional processing method in Chinese herbal medicine, with the effects of being insecti?cide and mildew proof and conducive to storage. Because of the low cost and the convenience in operation, sulfur fumigation has been abused in post harvest handling of herbs in recent years. In this paper,we discuss the effects of sulfur fumigation on the quality of Chi?nese herbal medicine, including the characteristics, chemical composition, heavy metals, trace elements and pharmacological activi?ties, and summarize the progress in several alternative technologies for sulfur fumigation,so as to provide a reliable basis for regula?tions and further research on sulfur fumigation and its alternative technologies.

ABSTRACT

To determine the efficacy and safety of Zhenjujiangya tablets combined with long-acting antihypertensive drugs in the patients with hypertension. Methods:A controlled study was carried out among 220 patients meeting the inclusion / ex-clusion criteria. All the patients were randomly divided into the experimental group and the control group. Both groups were treated with long-acting antihypertensive drugs as usual. The 110 patients in the experimental group were orally treated with Zhenjujiangya tab-lets ( bid) , while the other 110 patients in the control group were orally treated with Zhenjujiangya simulated tablets ( bid) , and the treatment course was 4 weeks. The efficacy was assessed using the reduction range of blood pressure, the clinical symptom score of hy-pertension and the indicators of ambulatory blood pressure monitoring as the indices, and the safety was assessed by the symptoms, signs and laboratory tests. Results:After the four-week treatment, the systolic blood pressure, diastolic blood pressure and hyperten-sion clinical symptom score were significantly decreased in the two groups, and the changes in the experimental group were more signifi-cant than those in the control group (P0. 05). After the treatment, the changes in average systolic blood pres-sure in 24h and daytime in the experimental group showed statistically significant difference (P0. 05), and the number of abnormal blood pressure in 24h and daytime in the experimental group exhibited reduction trend. There were no significant difference in the incidence of adverse events between the two groups (P>0. 05). Conclusion:Long-acting antihypertensive drugs combined with Zhenjujiangya tablets can further improve the antihypertensive efficacy without significant adverse reactions, which is suitable for clinical application.

ABSTRACT

OBJECTIVETo develop a quick identification method for the sun-dried and sulfur-fumigated Angelicae Sinensis Radix used by Fourier transform infrared spectroscopy (FTIR) combined with second derivative infrared spectroscopy.

METHODThe alcoholic and aqueous extracts of sun-dried and sulfur-fumigated Angelicae Sinensis Radix were analyzed by using FTIR, the further analysis was used by second derivative infrared spectroscopy.

RESULTThere existed differences between their infrared spectra either extracted by ethanol or water, while the distinctions were more obvious after analyzing their alcoholic and aqueous extracts through high resolution of second derivative infrared spectroscopy. Infrared spectra showed that the absorption peaks of Angelicae Sinensis Radix were significantly reduced and a new absorption peak appeared after sulfur-fumigated process in alcoholic extracts, while both of them changed markedly in the "fingerprint region" ranging from 1 000 to 400 cm(-1) in aqueous extracts. Second derivative spectra showed that the absorption peaks of sulfur-fumigated Angelicae Sinensis Radix extracted by ethanol weakened and disappeared at about 3 578 cm(-1) and 3 541 cm(-1), while both of them differed significantly from each other ranging from 1 400 to 1 200 cm(-1) as well as 800 cm(-1) to 600 cm(-1), difference also existed between them extracted by water ranging from about 3 900 to 3 850 cm(-1) and 3 800 to 3 750 cm(-1).

CONCLUSIONThe FTIS method combined with second derivative can be utilized to distinguish sun-dried and sulfur-fumigated Angelicae Sinensis Radix efficiently, conveniently and accurately, and provide a basis for identification and quality control of Angelicae Sinensis Radix.
